Community Participation

Support to get out, build confidence, join activities, attend appointments and stay connected to community life.

Community Participation support at home

Community participation support helps people do more than leave the house. It is about confidence, relationships, skill-building and being part of the places and activities that matter.

Every plan is shaped around the person receiving support. We take time to understand what a good day looks like, where help is genuinely useful, and how to preserve independence rather than take it away.

You can use these supports as a regular service, a short-term bridge after a change in health, or as a top-up alongside existing funded care. We keep pricing, schedules and responsibilities clear before services begin.
