A guide to switching home-care providers
Changing providers can feel daunting, but the process is more straightforward than most people expect. Here is how to switch without losing continuity of care.

People switch home-care providers for all sorts of reasons: moving suburbs, inconsistent rosters, rising costs or simply an approach that no longer fits. Whatever your reason, you have the right to change, and a well-planned switch should not leave a gap in your care.
Step 1: Get clear on why you are moving
Write down the two or three things you most want to be different. This keeps you focused when comparing new providers and gives your current provider a fair chance to fix problems — sometimes a frank conversation resolves the issue without a switch.
Step 2: Line up the new provider first
Confirm the new provider can cover your area, your preferred times and your specific supports before you give notice. Ask them to put a proposed start date and schedule in writing.
Step 3: Check your agreement and give notice
- Re-read your service agreement for notice periods and exit terms
- Give notice in writing and keep a copy
- Ask for any unspent funds or account balances to be reconciled
- Request copies of your care documents and history
Step 4: Plan the handover
A short overlap or a written handover summary helps the new team start well. Share routines, preferences, equipment notes and emergency contacts so nothing is lost in the move.
Rules about notice periods, exit processes and fund transfers vary between programs and change over time, so check the latest government guidance for the program you are funded under before you act.
